Complex Care, Community Care Manager:
Homecare Scotland is a well-established provider of Complex Care services to people living in their own homes. We provide personal care and social support for clients living with Brain Injuries, Spinal Injuries and Birth Injuries i.e., Cerebral Palsy. Clients receiving our support can live as independently as possible and enjoy a wide variety of social activities including holidays abroad.
Due to continued growth of our client numbers, we are delighted to be recruiting a Complex Care Manager. The successful candidate will be joining an established team based in our Glasgow office. Our team is supported by in house recruiters, trainers and senior managers.
Our ideal candidate will have experience in the care sector with an understanding of community based complex care provision, a customer focussed attitude and a sense of humour.
The Role Includes:
Client Management
· Regular client visits
· Care plan updates
· Risk assessments
· Rota management with team leaders
· Client reviews
· Liaise with ancillary services as needed i.e., GPS, Case Managers and District Nurses
· Work within current Care Inspectorate Guidance
Carer Management
· Recruit new carers as needed
· Arrange appropriate training
· Communicate with carers via telephone, face to face and email on a regular basis
· Ensure client rota is managed
· Spot checks/ supervisions and appraisals in accordance with Care Inspectorate guidance
Essential Criteria
· Recent Managerial Community Care experience
· Ideally SVQ 4 or nursing qualification
· Experience of caring for clients with complex care needs
· Understanding of Care Inspectorate Guidelines
· IT literate
· Driver with UK license own car
· Authorised to work in the UK
